ALL CHEERLEADERS - Saturday, July 25 - 10:00 to 11:00 am
A BLUE FOOTBALL - Saturday, July 25 - 10:00 to 10:30 am
B BLUE FOOTBALL - Saturday, July 25 - 10:30 to 11:00 am
C BLUE FOOTBALL - Wednesday, July 29 - 6:00 to 6:30 pm
D BLUE FOOTBALL - Wednesday, July 29 - 6:30 to 7:00 pm
D WHITE FOOTBALL - Wednesday, July 29 - 7:00 to 7:30 pm
E BLUE FOOTBALL - Wednesday, July 29 - 7:30 to 8:00 pm
E WHITE FOOTBALL - Wednesday, July 29 - 8:00 to 8:30 pm
MAKE UP DAY - Friday, July 31 - 6:00 to 7:00 pm
All football players and cheerleaders must have all paperwork submitted in order to receive equipment as well as begin practice on Monday, August 3rd. There will be no exceptions to this rule.
- Original birth certificate with the raised seal
- June 2009 final report card
- 2009 Medical forms – MUST BE DATED IN 2009!
* 2009 Pop Warner medical form - Page 1 completed by a parent
* 2009 Pop Warner medical form - Page 2 completed by a doctor
> A standard medical form will be accepted as long as it is dated 2009
> A written note from a doctor will be accepted with the following language “NAME is physically fit and can participate in Pop Warner football and cheerleading”. The note must be on a Doctor’s letter head and be dated in 2009.

FOOTBALL PLAYERS will be required to submit an equipment deposit check of $150.00
